HeartBridge Collective is a patient-led movement turning lived experience into leadership.
We’re patients, caregivers, advocates, and allies—coming together to change how the system sees, hears, and supports people living with heart valve disease and beyond.
Born out of survival, fueled by purpose, and grounded in both story and science—we exist to make sure no one walks this road alone.
Because the truth is: The system often saves your life… then leaves you to figure out how to live it.

Connect one-on-one or in small groups with other heart valve patients who truly understand what you're going through —because they've lived it too.
Personalized Guidance
Receive empathetic, experience-based support as you navigate your diagnosis, treatment options, and recovery-from people who've been there.
Participate in moderated discussions and storytelling circles that honor your journey and foster a sense of belonging.

The Match Process
Step 2
Welcome Email
You'll receive a welcome email with next steps while we find you a perfect match based on your answers from the "Get Matched" form.
Step 3
Your First Connection
Once matched, your mentor will reach out to you directly by phone within a few days to schedule your first conversation. This initial chat can take place over the phone, via video call, or through messaging—whichever you prefer.
During this first chat, your mentor will introduce themselves and take the time to listen to your story and answer any questions you may have. You'll work together to decide on the best ways to communicate and how often you’d like to connect. This is also a great opportunity to discuss your goals and what you hope to get out of the mentorship. Remember, there's no pressure; this conversation is simply a chance for you to get to know one another.
Step 4
Ongoing Support
Once you're connected to a mentor, your journey with HeartBridge continues with ongoing support and encouragement. Your mentor will check in with you as agreed, offering a listening ear, sharing helpful information, and providing resources. Additionally, we will introduce you to HeartBridge's online community groups and virtual events where you can connect with other patients to share experiences and feel less alone. Your feedback will also be periodically requested to help us improve the program for everyone.
